UCP Heartland enhances the quality of life of people with cerebral palsy and other disabilities, their families, and their communities. They believe that individuals with even the most challenging disabilities have the right to quality education and transition services, integrated employment opportunities with competitive wages and career opportunities. They also promote full inclusion in community life by providing resource and referral, advocacy, and community integration services.
This service ensures a consumer’s employment in the competitive environment is maintained in a long-term capacity. It is initiated once an individual’s Vocational Rehabilitation supported employment benefits have been exhausted or denied, and the individual’s employment support needs are at a frequency of 25% or less of his/her total hours worked.
This program provides short-term residential and emergency residential services to children and adults with developmental disabilities, who typically reside in their natural home. Services provide families with temporary relief from meeting the daily care needs of their loved ones in a safe place. The program is a facility-based, non-medical service in which individuals and their families can schedule needed care during the day, evening or overnight. Support needs range from comprehensive one-on-one assistance with all activities of daily living including personal care, eating, bathing and transfers, to less comprehensive assistance for people who are more independent, but still require supervision. Each consumer has an individualized plan that focuses on the acquisition of skills. Activities are planned to help participants achieve their goals.
