Accessing Case Management Services.


STEP 1 – Make Contact 

To determine if you or your family member is eligible to receive services, contact the Intake Department of:

STEP 2 – Determine Eligibility

The Intake Staff at the St. Louis Regional Office will set up an intake meeting. They will check medical and school records for a documented diagnosis of the individual’s developmental disability. If a recent evaluation is not available, the intake staff will conduct an assessment to determine if there are at least 2 substantial functional imitations in the areas of self-care, receptive and expressive language, learning, mobility, self-direction, and capacity for independent living or economic self-sufficiency.  

STEP 3 – Choose a Provider 

Once you are determined eligible for services you will be informed of your choices of providers for Targeted Case Management.  St Louis City residents three (3) years of age and older with active Medicaid are given a choice between case management services through St. Louis Regional Office or St Louis Office for Developmental Disability Resources (DD Resources). 

The standard for case manager caseloads  at DD Resources is a cap of 35 individuals per case manager.

Depending on your choice of case management services your case manager will be located at one of the following offices:
